Meet Steven Harlan: Philosophy and Approach
Steven Harlan, MD, stands as a singular figure in the evolving landscape of aesthetic medicine, where the convergence of artistry and scientific precision defines the modern practice of plastic surgery. From his earliest days in the Midwest, Dr. Harlan cultivated an appreciation for the human form that extended beyond mere physical appearance, recognizing that each patient’s sense of self‑worth and confidence is intimately tied to the way they perceive their own bodies. This insight fuels a philosophy that places the individual’s narrative at the heart of every surgical plan, insisting that no two procedures are ever truly alike. In his practice, the concept of “personalized rejuvenation” is not a marketing slogan but a guiding principle, compelling him to spend extensive time in consultation, listening to patients’ goals, anxieties, and life stories before ever entering the operating room. By integrating empathy with meticulous technical planning, Dr. Harlan creates a therapeutic alliance that transforms the often intimidating experience of surgery into a collaborative journey toward self‑actualization.
Beyond the patient‑centered ethos, Dr. Harlan’s approach is deeply rooted in an unwavering commitment to the ethical stewardship of surgical innovation. He views each new technology as a tool that must be rigorously evaluated, not only for its aesthetic potential but for its long‑term impact on tissue health, vascular integrity, and scar formation. This cautionary stance has led him to adopt a “measure twice, cut once” mindset, wherein detailed pre‑operative imaging, three‑dimensional modeling, and comprehensive risk assessment become standard components of his workflow. He constantly revisits the fundamentals of wound healing, tissue biomechanics, and regenerative biology, ensuring that his operative techniques are anchored in the latest peer‑reviewed evidence. In doing so, Dr. Harlan maintains a delicate balance between pioneering aesthetic outcomes and preserving the natural resilience of the patient’s anatomy.

Moreover, his mentorship of resident physicians and fellow dermatologic surgeons reflects a broader vision: to elevate the standards of aesthetic surgery across the region and to foster a new generation of clinicians who prioritize safety, artistry, and patient empowerment. This dedication to knowledge dissemination ensures that the philosophy he lives by today will echo in the operating rooms of tomorrow, creating a ripple effect that benefits countless individuals seeking transformative, yet responsible, cosmetic care.
Rigorous Medical Education and Training
Steven Harlan’s academic journey began within the hallowed halls of the University of Iowa Roy J. and Lucille A. Carver College of Medicine, where he earned his Doctor of Medicine degree with distinction. Immersed in a curriculum that emphasized both foundational science and clinical excellence, Dr. Harlan distinguished himself early on through a relentless curiosity about the mechanisms of tissue repair and the molecular pathways that underlie scar formation. His undergraduate research, conducted under the mentorship of leading faculty members, yielded multiple publications on dermal fibroblast behavior, earning him honors such as the Dean’s List and the prestigious Iowa Medical Scholars Award. These formative experiences cemented his resolve to pursue a career that would marry rigorous scientific inquiry with the hands‑on artistry of surgical practice.
Following medical school, Dr. Harlan embarked on a demanding residency in dermatology, where he honed his diagnostic acumen and refined his procedural skills. The residency program, renowned for its comprehensive exposure to both medical and surgical dermatology, provided Dr. Harlan with a unique platform to master complex reconstructive techniques and aesthetic interventions. He completed rotations in Mohs micrographic surgery, a discipline demanding exacting precision and an intimate understanding of oncologic margins, as well as in dermatopathology, where he developed an expert eye for histologic subtleties that influence surgical planning. Throughout his training, he consistently demonstrated an ability to integrate pathologic data with clinical decision‑making, a skill that would later become a hallmark of his aesthetic practice.
In addition to his formal training, Dr. Harlan pursued specialized fellowships that expanded his repertoire to include cutting‑edge laser technologies and minimally invasive facial rejuvenation. He was among the first cohort of physicians to receive advanced certification in CO₂ laser applications, a credential that required mastery of laser physics, tissue interaction models, and safety protocols. His fellowship experience afforded him the opportunity to collaborate with biomedical engineers, shaping the development of next‑generation laser devices that deliver precise ablation while minimizing thermal injury. This interdisciplinary exposure not only broadened his technical skill set but also reinforced his belief that the future of plastic surgery lies at the intersection of technology, biology, and patient‑focused care.
Surgical Expertise
Dr. Harlan’s clinical portfolio is distinguished by a breadth of expertise that spans from the microscopic realm of dermatopathology to the macroscopic artistry of facial reconstruction. In clinical dermatology, he excels at diagnosing and managing a spectrum of skin conditions, ranging from benign neoplasms to complex inflammatory disorders. His deep understanding of cutaneous physiology enables him to tailor treatment plans that respect the delicate balance of epidermal turnover, barrier function, and immune response. This nuanced approach is particularly evident in his management of patients with photodamage, where he integrates topical therapies, procedural interventions, and lifestyle counseling to achieve holistic skin health.
Mohs micrographic surgery represents a cornerstone of Dr. Harlan’s surgical repertoire, reflecting his commitment to oncologic safety and aesthetic preservation. By employing this tissue‑sparing technique, he is able to achieve cure rates exceeding 99% for cutaneous malignancies while conserving critical cosmetic units such as the nose, eyelids, and ears. The precision required for Mohs surgery—mapping, incremental excision, and immediate histologic examination—mirrors the meticulous attention to detail that Dr. Harlan brings to all aesthetic procedures. Moreover, his expertise in dermatopathology equips him to interpret frozen sections with unparalleled accuracy, ensuring that each margin is definitively clear before reconstruction proceeds.
Perhaps most emblematic of Dr. Harlan’s innovative spirit is his pioneering work with CO₂ laser facial resurfacing and CO₂ laser transconjunctival surgery. Since introducing CO₂ laser resurfacing to Iowa in 1995, he has refined the technique to address a wide array of facial concerns, including deep wrinkles, acne scarring, and dyschromia. The CO₂ laser’s ability to precisely vaporize epidermal and dermal layers while stimulating neocollagenesis allows Dr. Harlan to achieve dramatic rejuvenation with minimal downtime. In the realm of ocular aesthetics, his application of CO₂ laser transconjunctival surgery has revolutionized blepharoplasty, providing patients with smoother upper eyelid contours without the need for external incisions. These procedures underscore his mastery of laser-tissue interaction, his commitment to patient comfort, and his dedication to advancing the frontiers of aesthetic surgery.
Commitment to Patient Safety
Safety is the sine qua non of Dr. Harlan’s practice, a principle that permeates every facet of his clinical operations. As a member of the American Academy of Dermatology (AAD), he adheres to the organization’s rigorous standards for competency, ethical conduct, and continuing medical education. The AAD’s Code of Professional Conduct serves as a guiding framework for Dr. Harlan, ensuring that his interactions with patients, peers, and the broader community are grounded in transparency, respect, and evidence‑based practice. Through active participation in AAD committees, he contributes to the development of practice guidelines that safeguard patient outcomes across the nation.
Within Iowa, Dr. Harlan’s leadership extends to the Iowa Dermatology Society, where he collaborates with fellow physicians to promote statewide initiatives focused on patient safety, quality improvement, and public health education. He has spearheaded workshops on laser safety protocols, emphasizing the importance of proper eye protection, calibrated energy settings, and real‑time monitoring of tissue response.
